The psoas major — commonly known as the psoas — is the deepest muscle in your core. It connects your spine to your legs, runs through your pelvis, and is one of the most important muscles for posture, movement, and pain-free living.

When the psoas tightens or shortens — usually from years of sitting, repetitive movement, or compensating for an old injury — it pulls your lumbar spine forward and your pelvis out of alignment. The pain shows up in your back. But the source is the muscle pulling on it.

In some cases, the quadratus lumborum — a muscle deep in your lower back — is also contributing to the same pattern. Either way, the body is out of balance, and the back is paying the price.

This is what's been missed.

In about 7 out of 10 sciatica cases we see, the actual root cause is not the sciatic nerve itself — it's the psoas major muscle (commonly known as the psoas). The psoas is the deepest muscle in your core. It connects your spine to your legs and runs directly through your pelvis.

When the psoas tightens or shortens, it compresses the structures around the sciatic nerve and pulls your pelvis out of alignment. The pain shoots down your leg. But the source is the muscle pulling on the nerve.

In other cases, the piriformis — a small muscle deep in your hip — is the culprit. When it tightens, it compresses the sciatic nerve directly. That condition is called piriformis syndrome, and it mimics true sciatica almost exactly.

Either way, the nerve itself isn't broken. A muscle is squeezing it. That's what's been missed.

In some cases, the gluteal muscles or the IT band on the outside of your thigh are also contributing — usually because they've been compensating for the psoas dysfunction for years. Either way, the hip is out of balance, and the joint is paying the price.

The rectus femoris — the main muscle of your quadriceps group — runs from your hip down across your knee. When it tightens or shortens, it pulls your kneecap up and compresses the joint underneath.

The pain shows up in your knee. But the source is the quadriceps muscle pulling on the kneecap. Most knee pain isn't a knee problem. It's a quad problem with knee symptoms.

In some cases, the hamstring on the back of your thigh or the calf muscle below your knee are also contributing — usually because they've been compensating for the quadriceps dysfunction. Either way, the knee joint is out of balance, and the cartilage is paying the price.

This is what's been missed.

The pectoralis major and pectoralis minor — commonly known as the pectoral or "pec" muscles — sit across the front of your chest. When they tighten or shorten (usually from years of sitting at a desk, looking at a phone, or driving), they pull your shoulders forward and down.

To keep your head balanced, the muscles of your neck and upper back have to work overtime. The pain shows up in your neck, shoulders, and upper back. But the source is the chest muscles pulling everything forward.

In some cases, the trapezius or rhomboid muscles between your shoulder blades are also contributing — usually because they've been overworking to hold your shoulders back against the constant pull from the front. Either way, the upper body is out of balance, and your neck and shoulders are paying the price.
